Declaration
This is not a diagnosis service. This service is designed to support individuals with mental health concerns and enhance overall well-being through a variety of low-intensity interventions.
Handling, collection, storage and usage of data
The practitioner is a graduate member of the British Psychological Society (BPS) and adheres to its code of practice. All information is stored securely and confidentially in accordance with GDPR regulations. This includes clients' names, dates of birth, home addresses, next of kin, and GP details.
Privacy Statement
Session notes, and personal details are maintained in paper form within a secure, locked filing cabinet. All digital information is stored on a password-protected domestic computer to ensure confidentiality and security. Wellness and Guidance are committed to safeguarding your privacy and protecting your personal information and are registered with the Information Commissioner' Office (ICO).
Supervision and Quality Assurance
To ensure the quality of services, it is recommended that the practitioner engages in regular supervision. This may involve sharing certain information with the practitioner’s supervisor and/or accredited clinical sources who adhere to the ethical framework and guidelines of the profession. Topics discussed during sessions may be utilized to enhance the practitioner’s effectiveness and ensure the best possible support for clients. Your confidentiality is a priority, and all shared information will be handled with care.
Client Rights
Clients have the right to access their session notes. Any additional details held by the practitioner are intended solely for the practitioner’s own use and will not be shared. Your privacy is important, and Wellness and Guidance are committed to maintaining the confidentiality of your information.
Confidentiality
Confidentiality Policy
In accordance with UK law, the practitioner reserves the right to break confidentiality and disclose session information to relevant third parties if, in their professional opinion, the client’s safety is at risk—either to themselves or to others. This measure is taken to ensure the well-being and safety of all individuals involved.
Confidentiality Breach Procedure
If a breach of confidentiality becomes necessary, the practitioner will make every effort to discuss this matter with the client, and any recommendations will be documented. However, if discussion is not possible due to limitations or unforeseen circumstances, the practitioner may need to proceed with the breach without prior notice to the client.
Therapeutic Goals
The therapeutic goal is established collaboratively, with input from both the client and the practitioner. This goal reflects what the client aims to achieve as the desired outcome, ensuring that the process is tailored to the individual's needs and aspirations. The client is not tied into any long-term commitment, and they may end sessions any time.
Service Commitment
The practitioner will use reasonable care and skill in providing services. However, there are no guarantees of successful outcomes. It is not the practitioner’s responsibility to achieve the desired outcome for the client, and such outcomes cannot be guaranteed or promised. Making such guarantees would be unethical, as the therapeutic process is influenced by many factors, including the client's active participation.
Competence and Termination of Services
If the practitioner determines that the client’s needs exceed their level of competence, they reserve the right to terminate the therapeutic relationship. This decision will be discussed with the client during the session, and further recommendations will be provided to ensure the client receives appropriate support. Your well-being is the priority, and Wellness and Guidance aim to facilitate a positive path forward.
